What can home health care do for patients? Our services make it possible for eligible patients to receive quality skilled care in the comfort of their home. Home care can include

Skilled Nursing. Under the direction of a patient’s doctor, our registered nurses provide one-on-one compassionate care and teaching. Skilled Nursing services can include, but are not limited to:

  • Disease Management
  • Skilled long term care
    (Foley catheter care, injections)
  • Post operative care
  • Infusion Therapy
  • Colostomy care
  • Enteral feeding
  • Medication management
  • Patient education and caregiver instruction
  • Pharmacist consultation
  • WOCN Board Certified Wound Care Nurse
    • Prevent and manage pressure ulcers and non-healing surgical wounds
    • Provide assessment and product selection
    • Nutritional assessment
    • Pain and symptom management

Rehabilitation Therapy.  Our therapists work with patients with the goal of improving the quality of everyday living to achieve the highest level of independence through:

  • Physical Therapy
  • Occupational Therapy
  • Speech Therapy

Home Health Aides. The caring and experience Certified Home Health Aides on our staff provide help with personal cares and activities of daily living:

  • Bathing, grooming, dressing
  • Transferring and ambulation
  • Exercise assistance

Social Services. Our Medical Social Worker can help patients and families by coordinating necessary services
during and after home care:

  • Identify appropriate community resources
  • Provide counseling for patients and families regarding factors that affect independence at home
  • Long-term care planning

 

Who is eligible for home care? People of all ages can benefit from care in their home. To be eligible for services, patients must meet three basic criteria:

  • Under a doctor’s plan of care
  • In need of reasonable and necessary skilled care or therapy
  • Ability to leave home is restricted by illness or injury
