Licensed Medical Social Worker

If you are interested in a PRN position in the field of home health care, KVNA has an excellent opportunity to collaborate with our nurses and therapists.  Responsibilities of a home care Social Worker include but are not limited to assessing the nature of the patient's situation through interview and review of personal history, facilitating referrals to resources and programs to meet the patient's needs, developing appropriate plan of care, communicating with disciplines participating in the patient's care and ensuring thorough and accurate documentation.

Candidate must possess a compassionate and caring demeanor, excellent written and verbal communication skills and the ability to build a rapport with patients and their families.

  • Master's Degree in Social Work
  • One year experience in medical setting

6p-6a Correctional Health Nurse
